The sample illustrates a typical format for a subscriber's respondents.

  • Preventive issues that matter by age and gender
  • Significant limitations that matter for a particular respondent group - in this case functional limits and daily activities for the elderly.
  • Symptoms or problems
  • Other Issues - in the case of the elderly assistive devices and other health care providers
  • Age/gender breakdown of respondents
  • The Cumulative number of respondents who have significant limitations and the number who report clinician awareness, receipt of valuable education, and response to treatment, if given. (See also the diagram below).
  • Answers to the customized questions added to the survey by a subscriber.
  • SF-36 is a useful tool for applying national norms to these respondents. In general, a core higher than 70 is considered "healthy" and a score less than 55 indicates serious limitations.

A subscriber also receives this type of report the other age groups, for males and females only, and in comparison to other subscribers.

The PATIENT-PROVIDER INTERACTION SCORE is a very important marker of care quality and practice efficiency. It indicates the percentage of significant problems in a population of respondents that have been managed with appropriate education. The higher the score, the more likely the site is efficient and satisfying.

The diagram below for adults from 32 sites across the United States shows that the median score is near .5. This means that only 50% of the time have significant problems been addressed with excellent, very good, or good education and information.
